اللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنَ الْعَجْزِ وَالْكَسَلِ، وَالْبُخْلِ وَالْهَرَمِ، وَالْقَسْوَةِ وَالْغَفْلَةِ، وَالذِّلَّةِ وَالْمَسْكَنَةِ، وَأَعُوذُ بِكَ مِنَ الْفَقْرِ وَالْكُفْرِ، وَالشِّرْكِ وَالنِّفَاقِ، وَالسُّمْعَةِ وَالرِّيَاءِ، وَأَعُوذُ بِكَ مِنَ الصَّمَمِ وَالْبَكَمِ، وَالْجُنُونِ، وَالْبَرَصِ وَالْجُذَامِ، وَسَيِّيءِ الْأَسْقَامِ
O Allah, I seek refuge in You from helplessness and laziness, [from] miserliness and the old and infirm age, [from] hardness [of the heart] and heedlessness, [from] humiliation and destituteness.
And I seek refuge in You from poverty and disbelief, immorality, [from] shirk and hypocrisy, [from] seeking fame and attention, and I seek refuge in You from deafness and dumbness, [from] madness, vitiligo and leprosy, and [from] all bad illnesses.
N.B. Shirk means to associate others with Allah in those things which are specific to Him. This can occur in:
(1) belief, e.g. to believe that other than Allah has the power to benefit or harm without the will of Allah, or that they are equal to him in status or the merit to be worshipped beside Him,
(2) speech, e.g. to swear by other than Allah, which is a type Minor Shirk
(3) action, e.g. to bow, prostrate or to do any other acts of worship to other than Allah.

Allaahumma in-ni a‛oodhu bi-ka mi-nal-‛ajzi wal-kas-sal, wal bukh-li wal-ha-ram, wal-qas-wati wal-ghaflah, wadh-dhil-lati wal-mas-kanah
Wa a‛oodhu bi-ka mi-nal-faqri wal-kufr, wash-shirki wan-ni-faaq, was-sum‛ati war-ri-yaa’
Wa a‛oodhu bi-ka mi-naṣ-ṣamami wal-bakam, wal-junoon, wal-ba-raṣi wal-judhaami, wa sa-yi’il asqaam
Wa a‛oodhu bi-ka mi-nal-faqri wal-kufr, wash-shirki wan-ni-faaq, was-sum‛ati war-ri-yaa’
